New Patients

Please arrive 15 minutes early and fill out your paperwork. Rember to bring your insurance card.

Cancellation

Please call us at (612) 916-3506 prior to your appointment if you are running late or can't make your appointment. Failure to call prior to your appointment will result in a $20 charge.

Insurance  

As a courtesy we will attempt to verify your insuranc benefits for you, however, you are responsible for understanding  your coverage.  Call the number on your insurance ID card for specific issues like: 

• Deductibles, co-pays or visit limitations
• Coverage for services (chiropractic, massage therapy, acupuncture)
• Coverage for therapies, lab services, X-rays or diagnostic imaging

If you have an auto or work-related injury, report it to your insurance company or employer prior to contacting the clinic.

Medicare coverage for chiropractic care

Payment

Payment for co-pays and non-covered types of care is due at the time of service.
